Benefits Enrollment/Eligibility

This report provides detailed information on both enrollment and eligibility for benefit plans. You may use the enrollment results to reconcile provider statements or to create self-billings. The eligibility option provides information on all employees eligible for a plan regardless of whether or not the employee is actually enrolled in the plan at the specified time period.

 

401(k) Annual Census

This report provides a summary of all employees currently enrolled in the company’s retirement income deferral program. Information provided in the report includes retirement contributions, loans, and employer match data.

Enrollment Diagnostic Report

Use this report to identify all eligible employees who have triggered a specific diagnostic through the benefits enrollment process.

 

Dependents Report

This report displays all dependent information for an employee or group of employees.

 

Beneficiary Report

This report displays all designated primary and secondary beneficiaries for plans with beneficiaries (such as basic life and supplemental life).

 

Benefits Loss of Coverage

This report provides detailed information on loss of coverage events for benefit plans, as well as new eligibility and new enrollment.  The results can be used to assist in monitoring/managing COBRA, State Continuation, Conversion and/or Portability events.

 

Open Enrollment Confirmation Statements

Use this report to view one or more employees' Open Enrollment Confirmation Statements to see which benefits they elected during the open enrollment period.

 

Full-Time Equivalency

This report calculates the number of Full-Time Equivalent (FTE) employees working for your company in a selected month or range of months.

 

Income Affordability Analysis

This report compares an employee's medical plan cost to the employee's average monthly wages.  Use this report when ensuring that your organization's benefit offering meets certain affordability standards of the workforce.

 

Benefits Export File
