Yes/No Questions

This question type has participants select the answer Yes or No by clicking a radio button.

You can change the default answers to this question to easily create a True/False question or any other questions with only two possible answers.

Choice - One Answer Questions (Horizontal, Vertical, or Drop-Down)

This multiple choice question type has participants choose one answer from a list of possible answers. You may add as many possible answers as necessary.

Radio buttons corresponding to answers may be arranged horizontally or vertically, depending on your preference. Radio buttons may also be replaced with a drop-down box if you prefer.

Choice - Multiple Answers (Vertical)

This question type has participants check multiple checkboxes in response to the same question, allowing users to choose more than one answer. Checkboxes are arranged vertically and the participant may check none or as many as they wish.

Choice - Multi-Question Matrix

This question type has participants answer a series of sub-questions that all have the same possible answers, for example rating a list of company services on a scale of 1 to 5.

This question type has two sections, Responses (where you list the answers that all sub-questions will share) and Sub-Questions, where you list the questions that will share the answers you list in Responses.
