If you or your spouse has given birth to a child or completed the legal adoption of a child within the last 31 days, child and/or spouse may be added to your covered plans. You may also elect previously declined coverage for you and your eligible dependents. A Wizard will step you through the process necessary to complete the benefits change request. If you need to leave the process prior to completion, use the Exit button to save your changes and resume later.
Note: If the Life Event change request stays in Pending (unsubmitted) status for more than 14 days or is approaching the timed expiration date, you will receive a Message Center notification reminding you to submit it.

You have just returned from maternity leave and you want to add your new baby as a dependent on your benefit information. Since you will not be changing any of your benefit selections, you will not need to complete a life event. By adding a dependent to your file, you may select the dependent as a beneficiary or as a covered dependent during the next open enrollment period. |
To report a Birth or Adoption Life Event:
1. On the Menu, click Benefits > Report a Life EventBenefits > Report a Life Event.

The Life EventsLife Events page displays.

Note: You can also access the Life Events page from the Menu by clicking Quick Links > Report a Life EventQuick Links > Report a Life Event.

2. In the Create New Life EventCreate New Life Event table, click the Birth or Adoption link.

The Life Event WizardLife Event Wizard displays.

3. Enter a date (in the format mm/dd/yyyy) in the Effective DateEffective Date field.

Note: Use the child's date of birth or legal adoption date as the effective date. You must submit this Life Event no later than 31 days after the effective date, but you may not submit it before the actual event.
4. Specify whether this is a birth or an adoption in the Life Events NotesLife Events Notes box.

5. Attach any files in the Supporting DocumentsSupporting Documents section that may assist with validating the life event.

6. Click the Start WizardStart Wizard button.

The Add New DependentAdd New Dependent page displays.

7. To create a record for your new dependent, complete the detailed identification information on this page.
8. Click the SaveSave button to add the dependent and continue.

The Add New Dependent - Auto EnrollAdd New Dependent - Auto Enroll page displays.

Notes: The Auto-Enroll tool is available to immediately enroll your new dependent into your existing health plan coverage. The auto-enroll function is only applicable to health plans where you presently have existing coverage. You must indicate all other enrollment elections and changes (including new health plan selections, coverage changes for other members of your family, or enrollment to life or FSA plans) in the 'Change Elections' step(s) within this wizard process.
To use the auto-enroll, click the Enroll & ContinueEnroll & Continue button.

To bypass this auto-enroll option and make all coverage elections for this new dependent in the standard enrollment screens, choose the Skip Auto-EnrollSkip Auto-Enroll button.

9. If you click the Skip Auto-EnrollSkip Auto-Enroll button, the Life Event - Dependent ChangesLife Event - Dependent Changes page displays.


Notes:
The displayed table contains a list of all dependents presently included in your benefits elections. You may make changes to your dependent list on this screen if necessary.
To report an additional birth/adoption, use the Add DependentAdd Dependent link to create all necessary dependent records.

10. Enter information for any other dependents in your family who will be newly added to your coverage due to this event.
Note: You may not delete dependents as part of a birth/adoption Life Event. However, if your dependent will be obtaining coverage elsewhere, you may unenroll them. The UnenrollUnenroll link will remove the dependent from all existing coverage.

11. After completing all dependent changes, click the NextNext button.

The Life Event Wizard (Information)Life Event Wizard (Information) page displays.

Note: In the steps that follow, the system will display a summary view of your current year elections.
12. Click the NextNext button when satisfied with the summary.

13. To make the appropriate changes to your current year coverage, click the ChangeChange button corresponding to the relevant plan.

Note: Be sure to include all desired changes for this Life Event before continuing on to the next step. This would include enrolling new dependents into a particular plan, changing plans, or waiving coverage previously selected.
14. After completing all Current Year elections, click the NextNext button.

15. Click the NextNext button.

The Life Event Enrollment ValidationLife Event Enrollment Validation page displays.

Note: The Validation process will verify that all modified elections meet certain enrollment rules according to your plan rules. If critical errors are encountered, they will be indicated in this report. You must correct these errors before submitting the Life Event.
16. Carefully review all information on this report.
17. If you need to adjust any information, click the PrevPrev button and return to the Benefit Enrollment screen.

18. If you are satisfied with these elections, click the PrintPrint button to obtain a printed copy of this report.

19. Click the NextNext button.

The Life Event Wizard (Information)Life Event Wizard (Information) page displays.

20. Click the NextNext button.

The Life Event SummaryLife Event Summary page displays all changes to your benefits.

To edit or view detailed Life Event elections, click the EditEdit button.

To leave the Life Event open allowing you to make further adjustments before submitting the Life Event change request, click the ExitExit button.

To permanently cancel changes prior to execution, click DeleteDelete button.

21. If all changes are accurate, click the SubmitSubmit button to forward your Life Event to the benefits administrator for approval.

Notes:
Changes will not occur until your Life Event is approved by the benefits administrator and the designated effective date is reached. Once approved, all changes will be reflected in your records on the approval date, or effective date, whichever is later.
You may need to forward evidence of your qualifying Life Event to the benefits administrator.
